Dear Anthem BC/BS patient,

We are happy to accept your insurance plan. Our insurance specialist will assist you prior to your first visit; we will submit all insurance claims on your behalf.

1. On your initial consultation, our staff will pre-approve and certify all insurance claims directly to your insurance company.
   
   
2. Furthermore, we will submit and process all insurance claims for you.
   
   
3. You will be responsible for co-pay and deductible as listed in your policy. Please call our staff; they will be happy to explain co-pays and deductibles to you.
   
   
4. Following your initial consultation, our staff will prepare a comprehensive verification of benefit (VOB) with your insurance company. We will provide you a copy of this valuable document.
