Dear BCBS-HMOI patient,

We are happy to accept your insurance plan. Our insurance specialist will assist you prior to your first visit; we will submit all insurance claims on your behalf.

1. We accept all related insurance plans BCBS, BCBS-HMOI
   
   
2. Please be aware you will need a referral prior to your initial consultation. Call our staff if you need help obtaining a referral.
   
   
3. On your initial consultation, our staff will pre-approve and certify all insurance claims directly to your insurance company.
   
   
4. Furthermore, we will submit and process all insurance claims for you.
   
   
5. You will be responsible for co-pay and deductible as listed in your policy. Please call our staff; they will be happy to explain co-pays and deductibles to you.
   
   
6. Following your initial consultation, our staff will prepare a comprehensive verification of benefit (VOB) with your insurance company. We will provide you a copy of this valuable document.
   
   
7. Each treatment cycle will be pre-certified with HMOI.
